In consideration of the All Cal Hockey League allowing me to play in its hockey league, I, ____________________ for myself, my heirs, executors and administrators release the All Cal Hockey League, All Star Sports Management, Fred Zdanowicz, its respective servants, agents or employees from any claims, demands, damages, actions or causes of actions arising out of or in consequences of any loss, or damages to my person or property occurring while attending at or participating in any hockey game or activity in any way associated with or sponsored by the All Cal Hockey League, not withstanding such loss, injury or damages may have arisen by reason of the negligence of the All Cal Hockey League, All Star Sports Management, Fred Zdanowicz, its servants, agents or employees.  I also agree to abide by the bylaws, playing rules and decisions of the All Cal Hockey League.  I am aware that these bylaws and playing rules are available to me through my team management.
